Getting started:

1. Create your account

Click on the button to set up your profile in our parent portal. You will need pictures of your insurance cards to upload.

2. Complete paperwork

Click and fill out these forms:

Read and keep these for your files:
Notice of Privacy Practices

*(updated 12/2024)*

3. Ask your doctor to send a prescription

Please call your pediatrician and request that they fax a prescription requesting evaluation and treatment for all services you are requesting and include any relevant diagnostic codes (autism, developmental delay, etc.).

Next Steps:

You will get an email with the subject line “Welcome to KidPRO” with prompts to set up your password. Check your spam folder. Please set this up and login to your account.
